Managing your health online
Engage Consult
We use a system called Engage Consult to receive messages about appointments, admin updates, and much more. For more information about Engage Consult, please visit our “Appointments” page, or read our news articles for more information.
NHS App
You are able to view your health records in the NHS App, as well as order repeat prescriptions, view upcoming appointments, and submit requests through Engage Consult. For more information about the NHS App, please visit the NHS UK website.
Other ways to manage your health online
There are many different third-party providers that you may use to manage your health online, such as Patient Access. These are not provided by the NHS, and most issues with the services will need to be directed to the providers.
Viewing your medical records online
You are able to view your health records in the NHS App, on Patient Access, or any other online health record management service. Every patient should be able to view their records in the NHS App from October 18th 2023 onwards (or from the date you registered if you have registered with us after this date).
If you are unable to view this information in the NHS App, Patient Access, or any other online health record management service, it may be that there is an issue with your online services account. To create or amend your online services account, we may need to ask that you complete an online access form. These forms are separated into two categories: Basic and Advanced.
Basic online access request
A Basic online access request form can be found on Engage Consult. Completion of this form will grant you access on the NHS App to:
- Contact us via Engage Consult (including booking appointments)
- Order repeat prescriptions
- Change pharmacy nomination
- Receive and read messages from the Practice
- View your immunisation record
- View your medicines list
- View your allergies and adverse reactions list
- View your test results
If you would like this level of access to your digital health record, please complete the Basic access form on Engage Consult. Please note: this request will only show what records we hold for you digitally. If you require sight of your paper records, please complete a “subject access request”.
You will be asked to submit a photo of yourself holding photo ID next to your face for verification purposes. This is to ensure that the request is valid. Please attach your photo when prompted to at the very end of the questionnaire. If this is not possible, please state this at the end of the form.
Once submitted, we will begin processing your records for you. We will contact you when your access has been granted. It may be several weeks before you hear back from the team. If you have not heard from the team after four weeks, please contact the practice to request an update.
Advanced online access request
An Advanced online access request form can be found on Engage Consult. Completion of this form will grant you access on the NHS App to:
- Contact us via Engage Consult (including booking appointments)
- Order repeat prescriptions
- Change pharmacy nomination
- Receive and read messages from the Practice
- View your immunisation record
- View your medicines list
- View your allergy and adverse reactions list
- View your test results
- View your consultations and events
- View your documents
- View your health conditions (sometimes called ‘problems list’)
If you would like this level of access to your digital health record, please complete the Basic access form on Engage Consult. Please note: this request will only show what records we hold for you digitally. If you require sight of your paper records, please complete a “subject access request”.
You will be asked to submit a photo of yourself holding photo ID next to your face for verification purposes. This is to ensure that the request is valid. Please attach your photo when prompted to at the very end of the questionnaire. If this is not possible, please state this at the end of the form.

Viewing health records for others online
You may view and manage the health records for others online. This access is requested through a Proxy Access form on Engage Consult. Completion of this form will allow you to (on their behalf):
- Contact us via Engage Consult (including booking appointments)
- Order repeat prescriptions
- Change pharmacy nomination
- Receive and read messages from the Practice
- View immunisationrecord
- View medicines list
- view allergies and adverse reactions list
- View test results
This form will ask for the information of the person who’s records you wish to access (the “Donor”), as well as your information (the “Representative”).
Please read the important information below before submitting a Proxy Access form.
Proxy Access for someone over 16 with capacity to grant consent
If you would like to access the records of someone over sixteen who has the capacity to grant consent for you to do so, you will need to complete a Proxy Access form on Engage Consult. Please select “the patient has the capacity to consent to grant proxy access” when asked.
Please submit as requested an image of you holding photo ID next to your face at the end of the form. Please also submit an image of the person who’s records you wish to access holding photo ID next to their face. If any of the above, please state this at the end of the form. This is for verification purposes.
We will then review your form and process accordingly. We will contact you if we need any more information. Please note: we may contact the person who’s records you wish to access to check their consent.
Once granted, this access will remain active indefinitely, or until it is closed due under special circumstances (e.g., revoking consent).
Proxy Access for someone over 16 without capacity to grant consent
If you would like to access the records of someone over sixteen who has the capacity to grant consent for you to do so, you will need to complete a Proxy Access form on Engage Consult. Please select “The patient does not have the capacity to consent to grant proxy access” when asked.
Please submit as requested an image of you holding photo ID next to your face at the end of the form. Please also submit an image of the person who’s records you wish to access holding photo ID next to their face if this is possible. If any of the above not possible, please state this at the end of the form. This is for verification purposes.
We will then review your form and process accordingly. We will contact you if we need any more information.
Once granted, this access will remain active indefinitely, or until it is closed due under special circumstances (e.g., revoking consent).
Proxy Access for someone under 11
If you would like to access the records of someone under eleven, you will need to complete a Proxy Access form on Engage Consult. We can grant this access to anyone who is a legal guardian for the patient who is under eleven. As the person who’s records you wish to access is under eleven years old, they are defined in legislation as not having the capacity to give consent to grant access to their medical record. Therefore, please select “The patient does not have the capacity to consent to grant proxy access” when asked.
Please submit as requested an image of you holding photo ID next to your face at the end of the form. Please also submit an image of the child’s birth certificate, adoption certificate, or any other certification that states that you are their legal guardian. If any of the above are not possible, please state this at the end of the form. This is for verification purposes.
We will then review your form and process accordingly. We will contact you if we need any more information.
This access is granted to the representative until the child turns eleven. Access is revoked at this age due to protecting the privacy of the child who may now have the capacity to consent to grant access to their records. For proxy access to a child’s records after this age, please see the guidance below: “Proxy Access for someone between 11 – 16”.
Proxy Access for someone between 11 – 16
If you would like to access the records of someone aged between eleven and sixteen, you will need to complete a Proxy Access form on Engage Consult. Some children between eleven and sixteen may be competent to consent to grant proxy access. Please select the best option when prompted regarding their capacity to consent to grant proxy access. We will review this when processing your request.
Please submit as requested an image of you holding photo ID next to your face at the end of the form. Please also submit an image of the child’s birth certificate, adoption certificate, or any other certification that states that you are their legal guardian. Additionally, please also send through an image of them holding photo ID next to their face. If any of the above are not possible, please state this at the end of the form. This is for verification purposes.
We will then review your form and process accordingly. We will contact you if we need any more information. Please note: we may also contact the child who’s records you wish to access to check their consent.
This access is granted to the representative until the child turns sixteen, or sooner if it is closed due under special circumstances (e.g., revoking consent). Access is revoked at this age due to protecting the privacy of the child, who may not consent to grant access to their record beyond turning sixteen.
