This company will sometimes approach us for reports to support applications for insurance or mortgages. They have adopted a policy which does not include seeking signed consent from you. We are not able to provide information about you to a third party without your signed consent. We are also not able to act as administrators for Legal and General. We have advised this company that before they seek reports from us they must either provide signed consent with the request or ensure that they have asked you to provide this.

Please do not ask our patient services team to provide forms for you to sign. If they contact you asking you to do this, you should ask them to email or post a consent form to you to sign and send on to the practice.
